The North Carolina name change for an adult as descibed in the State Code Chapter 101 is done by filing a petition to the county's civil court where you are a resident. It is easier, if you were just married, to have your certified marriage license with the raised seal or if you were recently divorced to have your divorce decree. Typically, you will not have a court hearing in North Carolina but it depends on the circumstances of the name change.
REQUIREMENTS
Must be a resident for at least 6 months
If just married, must have your original marriage certificate.
If just divorced, must have your divorce decree.
Step 1 - Fill-Out the Name Change Petition for North Carolina. The Petition must be notarized. Also, you will need the North Carolina Affidavit of Good Character.

Step 2 - Make 2 copies of each.

Step 3 - File the Petition to the County Court Clerk's office in the County where your live. You will need to pay the required filing fee (different in every county) and file the following documents:
Name Change Petition
Name Change Affidavit of Good Character
Documents proving your residency such as a driver's license or utility bill
A list of where you have lived for the past 10 years.
Step 4 - You will be assigned a court date. At the hearing, the Judge will authorize your order granting name change and your name change decree which will be your official form for changing your name at any public or private institution.
Step 5 - Under North Carolina law upon receiving your new name change you must now go out and change your name at every public and private establishment that should be aware of this change. This includes but is not limited to: DMV (Driver's license), Passport Agency, Social Security Office, Banks, Insurance Companies, etc.
