How to Complete your TUberculosis (TB) Requirement

Welcome to the UCSB student health website
Today I will be showing you how to complete Your Tuberculosis Health Screening requirement.
Please click on Gateway secure log in
Select to log in as a student or an authorized representative
You will be prompted to enter your UCSB net id and password once these fields are complete, select proceed.

This will bring you to your home page, please look at the left column and select forms.

Required entrance forms.

The form we will be focusing on today will be Tuberculosis (TB) Health assessment and Tuberculosis Screening questionnaire.
If you are required to complete the Tuberculosis (TB) Health Assessment form please download the form below
Take this form to your local medical provider to complete or document TB testing
Once this form is completed and signed, return to this portal and select immunization upload where you can upload your TB document.

By submitting this form you are acknowledging that you understand that you must return to the SHS gateway portal to upload your form for TB compliance.
If you must complete our TB health screening questionnaire please answer questions 1-5, submit the form and check your gateway portal secure messages regularly to see if you require to submit TB testing.
Please be sure to complete this form as accurately as possible as any errors may interfere with your registration process.
Once complete you can select submit final.
