In this video I show to to create a Google Form from a Google Spreadsheet with a script I constructed from things I found on the internet. I am not an expert in this. I just copied, pasted, and moved some things around. The link to the public script which you can copy is here: goo.gl/ExyUW1

If you are receiving an error with the image, you will just have to omit it. There must be a bug with Google's "getUrl" function, which I have no idea how to fix. This link here is the same script without the image item option to make it simpler: goo.gl/lTilgA
