Intstructions on how to change your name in the State of New Jersey for any resident seeking a name change after marriage, after divorce, or for personal reasons. You must start the process by filling out the name change packet provided by the State.

Name Change Packet. - Download the name change packet provided by the State. The kit provides the following forms:
Civil Case Information Statement
Verified Complaint for Name Change & Certification
Order Fixing Date of Hearing
Notarize. - You must fill out the forms and sign in front of a notary. You can find a notary at every branch bank.
Submit Forms. - Submit the forms to the county court clerk's office where you reside.
Newspaper Publication. - The clerk will direct you to the newspaper where you will have to make an ad for publication. In New Jersey, you must announce your new name publicly, and putting an ad in the newspaper is the only way to complete this task.
Upon completion of newspaper ad running in circulation you will be provided a 'Notice of Publication' that will prove to the court that you successfully made your name change public.
Hearing. - You must go to your hearing with the 'Form of Judgement' form (included in kit) and your 'Notice of Publication'. The Judge may ask you why you wish to change your name and upon completion of hearing you will be granted your name change decree.
You can use your name change decree to change your name at any private or public institution.
