How to File A Gap Claim When You Have Kaiser in 5 Steps. We understand this process may be confusing, so we put together this video to keep it simple!

Step 1 -- Wait until the end of the month for which services were rendered. Kaiser will reconcile your bills at the end of each month.
Step 2- Once a new month has arrived, call Kaiser to request a "Demand Statement" for the month services were received. Kaiser will mail this to you. This is the most important document needed to file a claim. Here are the phone numbers:
Step 3 - If you paid for services while at Kaiser, you will not have a bill. If you didn't pay at Kaiser, you will have a bill for services. Collect this bill in preparation for sending it to Assurant.
Step 4 -Complete an Assurant Gap claim form. This form asks you to "Describe Injury Sickness Completely." Be sure to explain the claim in detail.
For example: "Lab Tests" is not descriptive enough. You will need to write "Lab tests recommended by the doctor as a result of pain in my stomach."

Step 5- Collect these document:
1) A Demand Statement
2) Your Bill from Kaiser (if available)
3) Your Completed Gap Claim Form
