How to Use a Custom Functional Capacity Evaluation Form to Win Your Disability Case

Since Social Security defines “disability” in terms of how your medical problems and associated treatment (like medication side effects) prevent you from performing the duties of a simple, entry-level job. A detailed functional capacity form completed by your doctor can be an indispensable tool to present to the Social Security judge.

In this video, I show you the official #SSA functional capacity form and I explain why this form is flawed and won’t help you much. I also show you an example of a custom form that addresses the specific work activity issues relevant to a Social Security judge.

In my practice I would customize this “custom” form even more to make it specific to your medical records and you exact situation but even in its unmodified state the custom form I found online is far superior to SSA’s official form.
