There has been some confusion about how to fill out the v55/5. This is how i have imported my Yamaha xs650 into the uk and applied for a uk registration and V5C.

Forms you will need.

Completed V55/5
The American title for the bike.
Passport / or some kind of ID
Bank or council tax statement.
Money's for Tax 6 months or 1 year and £55 for the application.
MOT certificate.
If you have all of these then your good to go.
