HOW TO LEGALLY CHANGE YOUR NAME IN CALIFORNIA (PRO PER)

1. Pick a name

2. Get/find a copy of your birth certificate: cdph.ca.gov/pubsforms/forms/ctrldforms/vs111.pdf

3. Fill out forms (FORM GUIDE: goo.gl/CZtKqw)

Order to Show Cause: courts.ca.gov/documents/nc120.pdf

4. File your forms with the clerk and pay fees ($425), and get your court date (consider timing).

5. Publish in a local paper for four weeks and pay (not required for gender identity cases).

6. Go to court and pick up your decree and pay for certified copies ($25.50 each in SCC).

7. Change with social security: ssa.gov/forms/ss-5.pdf

8. Change with DMV ($29)

9. Change with Passport

11. Change with bank. Bring court order, ID (some banks may require new ID), bring anyone who you have a joint account with (spouse, parent, business partner, etc.).

12. Change with work/school (every work place/school has different policies but you will probably need your court order and ID).
