UPDATE: This version is outdated. You can now open video files directly into Photoshop CC and convert frames to layers and go from there. It's much more simple.

Hopefully this video helps. It really is an outdated technique but for those of you still using CS5, this technique still works.

I hope to make a CS6 tutorial soon. The process is much easier.

I'm sorry in advance if you do not have the two programs that I have. They're not always free. :/ I am also unsure which freeware works the best for these two operations.
