To get your Device ID, follow these instructions:
1) Connect your device to your computer
2) Launch iTunes
3) Select your device in iTunes
4) In the first pane "Summary", click on the "Serial Number", and it will become "Identifier (UDID)"
5) Paste this code in the form (you can right click and copy it)
That's it!
