Here's what you will need:
- Application Form DS-11;
- Evidence of U.S. Citizenship;
- Proof of Identity with a photocopy;
- Proof of travel;
- Passport photo;

Just follow these steps:

Step 1: Report the Lost Passport:
Call 1-877-487-2778 to report the lost passport.

Step 2: Complete Passport Form DS-11:
Do not sign the application until instructed to do so by the Acceptance Agent.

Step 3: Gather Supporting Documents:
Provide evidence of U.S. citizenship such as a certified birth certificate or previously-issued, undamaged passport;
Present a valid photo ID such as driver's license or Naturalization Certificate;
Include a copy of your itinerary or e-ticket as proof of travel;
Submit a standard passport photo taken within the last 6 months.

Step 4: Visit Application Facility:
