How to remove objects from behind subjects in Photoshop

Ever tried to remove items from behind the subject? Like trees, bokeh circles, arms or other distractions?! Well, I use this method all the time because I usually have a hair mask to hand and it's so easy - no need to be careful about the edges when cloning away!
