Dried nettles: book 1, ch. 8, moment 3 "Hagrid's Wooden House"
or: book 2, ch. 1, moment 2 "Two Enormous Green Eyes"
Bat Spleens: book 1, ch. 10, moment 3 "The Mountain Troll"
Pufferfish Eyes: Diagon Alley

Once you have completed this potion you can go back to Snape's Office (book 2, ch. 5, moment 3) to steal ingredients for the Polyjuice Potion.
