Instructions to change your name in the State of Minnesota for any resident seeking to change his or her name after marriage, after divorce, or for personal reasons. You must be a resident for at least 6 months.

Fill-Out Forms. - You will need to completely fill out the following forms:
Name Change Petition
Criminal History Check Release
Order Granting Name ChangeSTOP at section titled 'IT IS ORDERED'

Notary. - You will have to sign your forms in front of a notary public.
Mail or Hand-Deliver Forms. - You will need to mail or hand deliver the form to the Court Nearest You. You will have to call and ask the filing fee amount.

Court Date. - You will be notified of your hearing date or the date at which your name change decree will be available. If you were just married or divorced, chances are you will not be asked to appear in front of the Judge.

Change Your Name. - With your name change decree you must notify all institutions and establishments that you do business. You must make them aware of your name change.
