SAMPLE LETTER REQUEST FOR BUSINESS CLOSURE (BARANGAY)

PERMIT NO.) issued on (DATE ISSUED) and Business Permit No. (YOUR BUSINESS PERMIT NO.) issued on (DATE ISSUED) by City Government of (YOUR CITY OR PROVINCE). That I am applying for closure of business effective this point on due to (REASON FOR CLOSURE). The said business is no longer operational since (DATE CLOSED). That I settled my obligations. That I am executing this letter to attest to the truth of all the foregoing statements and for whatever legal purpose it may serve signed this 10th of March 2020.

BIR Form No. 1905 (2 copies)- (I forgot to mention this on the video) BIR will provide the sheet
Inventory of unused sales/official receipts- BIR will provide the sheet
Unused sales invoices/official receipts
C.O.R Form 2303 Original
Ask for Receipt Sign
Barangay Clearance or Certificate of Closure
Municipality Clearance or Certificate of Closure
*In case of authorized representative who will transact with the BIR in behalf of the taxpayer,
Special Power of Attorney is a requirement.
