How to Form an LLC in the State if Utah under the state statute 48-2C-403 that allows a resident to apply for a domestic Limited Liability Company through an Articles of Organization, paying a filing fee, and sending to the Secretary of State.

Steps
1 Choose the Name. - The first step is to choose a name for your LLC. Be sure to make it a name that fits your industry and is something that is brand-able.
Check to make sure your name is available by searching the Utah LLC Database.
2 Get Utah LLC Articles of Organization. - You will need to download and complete this form either on your computer or printed out in neat black ink.
Pay filing fee of $70 in check payable to the 'Utah Secretary of State'.
Send filing fee and documents to: Utah Division of Corporations & Commercial Code, PO Box 146705, Salt Lake City Utah 84114-6705
3 Wait for LLC in Mail. - The State of Utah typically processes LLC's within 5 to 7 business days of receiving the application and filing fee. The secretary of state will send your official documents to the mailing address of the principal office.
4 Apply for Tax ID Number. - Or referred to as an Employer Identification Number, this allows you to open a bank account for your Company.
